Outpatient Physical Therapist

POSITION SUMMARY:
This role is responsible for providing medically prescribed, goal-oriented physical therapy in a patient’s home or select location based on treatment/patient needs. The role includes providing therapeutic treatments and participating in the development of patient physical therapy plans of care.
PRINCIPAL ACCOUNTABILITIES:
Perform initial and ongoing physical therapy evaluations and re-evaluations according to APTA standards of practice as well as state standards.
Develop a plan of care/treatment plan for each patient based on the initial evaluation and objective findings, including short and long-term goals, keeping the patient and caregiver as the focus of care.
Implement treatment plan on an established schedule and adjust this schedule and plan according to patient progress.
Document patient progress according to approved medical documentation and facility documentation procedures.
Evaluate mobility, strength, and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and care realistically attainable.
Report patient progress to the patient, patient caregivers and healthcare professionals involved in the ongoing treatment/care of patients.
Develop a plan for patients following completion of their Plan of Treatment with appropriate support and follow-up services if needed.
Maintain professional and technical knowledge through attending educational workshops, reviewing professional publications, etc.
QUALIFICATIONS:
Compassionate and able to deliver quality patient-centered care

Motivated and able be excel in a self-managed environment
Desire to work a flexible schedule based on patient needs
Ability to travel to patient homes or the desired location for treatment
EDUCATION:
Bachelor’s Degree in Physical Therapy from an accredited physical therapy program.
CERTIFICATIONS, LICENSES AND REGISTRATIONS:
Currently-licensed Physical Therapist in the state of Wyoming
Possesses and maintains a current CPR certification
Valid driver’s license and auto liability insurance

Position Summary
We are seeking a compassionate, motivated Physical Therapist Assistant (PTA) to join our team. The PTA will work under the direction of licensed physical therapists to provide exceptional care across multiple settings/ environments. This role is ideal for a clinician who enjoys variety, problem-solving, and working with patients of all ages and abilities.
Key Responsibilities
Implement individualized treatment plans established by supervising physical therapists.
Deliver interventions for a variety of patient populations, including:
Orthopedic conditions (post-surgical, musculoskeletal, sports injuries)
Neurological diagnoses (stroke, Parkinson’s, MS, balance disorders)
Women’s health (pelvic floor, pre-/post-natal, osteoporosis)
Fall prevention/ balance and functional mobility training
Support community-based services such as:
Home safety and accessibility assessments
Wheelchair seating and positioning evaluations (with PT guidance)
Document patient progress accurately and in a timely manner in the EMR.
Collaborate with PTs and other team members to adjust care plans and ensure continuity of care.
Provide education to patients, families, and caregivers on exercise programs, equipment, and self-management strategies.
Participate in hybrid service delivery as appropriate.
Maintain compliance with federal, state, and clinic policies and uphold professional standards.
Qualifications
Graduate of an accredited Physical Therapist Assistant program.
Current [State] PTA license in good standing (or eligible for licensure).
CPR/BLS certification.
Strong interest in one or more of the following areas: orthopedics, neurology, women’s health, geriatrics, or community-based rehab.
Excellent communication, organizational, and interpersonal skills.
Ability to work independently and as part of a collaborative team.
Comfort with technology (EMR, telehealth platforms) preferred.
Valid driver’s license and reliable transportation for community/home visits.
Preferred Experience
Outpatient clinical experience (orthopedic, neuro,balance/ fall, etc).
Experience with balance and fall prevention, home assessments, or adaptive equipment fitting (training provided if needed).

Rehab Tech
JOB TYPE: 10-20 hours/week (Part Time Position)
*Goal of Purpose Physical Therapy is to transition this position to a full time position as more clients sign up for services
POSITION SUMMARY:
Physical Therapy tech works under the supervision of the supervising Physical Therapist. The tech’s role is to support the patient by providing supervised exercise and functional movement activities to allow them to remain as independent as possible in their current living environment. Purpose PT wants to ensure each of our patients has the opportunity to continue their functional movement training to allow them to stay as independent as possible.
PRINCIPAL ACCOUNTABILITIES:
Keeps accurate time keeping for each visit
Schedules their clients independently
Provides maintenance exercise and functional movement tasks which can include ROM, strengthening and gait/balance tasks.
Practices safe movement and patient handling with all clients.
Communicates with supervisory PT and caregivers of the client as needed.
